What was originally intended as an expansion pack for the granddaddy of the beloved first-person-shooter genre: Wolfenstein 3D, Apogee Software's old-school FPS title Rise of the Triad expanded in scope, becoming a sequel to Wolfenstein, and eventually evolved into its own IP.
